How much will my copayment be?
Your co-pay amount should be listed in your insurance plan documents or even on your insurance ID card. If you can't find it, you should be able to find out the amount of your co-pay by calling the customer service number on your insurance ID card. Be aware that your co-pay amount can vary depending on the particular medical service you are receiving.
What is your new patient procedure?
To schedule, cancel, or modify an appointment, you may call our office directly. Clinic staff are available by phone Monday - Thursday, 8 a.m. - 4 p.m, and Friday, 8:00 a.m. - 12:00 p.m. Please call at least 24 hours in advance if you need to change your appointment so we can schedule another patient at that time. We ask that you arrive 15 minutes early for an office appointment to update forms as necessary.
I don’t have my insurance card with me, can I still receive treatment?
While we do accept cash pay, it's important that we have all relative information according to your plan. If at all possible, it is best to have the following information to give to our front desk staff:
1. Member ID
2. Group/Policy Number
3. Plan Name/Type
